Open Enrollment for Blue Pathway^SM Child-only Policy Runs through February 29, 2012

Open enrollment for Blue Pathway, our health insurance plan for children who do not have other major medical coverage available to them for purchase, began January 1, 2012 and runs through February 29, 2012.

• Plan for applicants ages 1 through 18 years old¹
• Coverage for emergency care, office visits, hospital stays and prescription drugs
• Preventive care services² covered at 100%

Blue Pathway provides guaranteed acceptance for applicants who are at least one year old and under 19 years of age.

Product details and enrollment materials will be posted on our website beginning January 1, 2012.

¹ Children eligible for these plans will be approved for coverage, regardless of health status. Applicants must still meet any non-health related eligibility criteria and must not have other major medical coverage available to them for purchase.

² Benefits covered as defined by national guidelines. Must use in-network providers to receive maximum coverage.
